A client saw this plugin on another site and asked me to install it. It was easy to setup and get working, I really like that Admin must approve a submitted rating before public view. I used the «amr shortcode any widget» plugin to load the Cosmick Star Rating widget into my form submission page so I could control how many ratings were displayed above the form.
